Illinois Medical Compassionate Care is patient-friendly, and we can help you with the medical certification and the required photograph. To get the medical certification, we will need medical records that support a qualifying diagnosis (Click to see qualifying conditions). We can also take your picture for free. All of these items will be emailed to you after you complete your evaluation by our medical professionals. Evaluations are done through two visits.

Yes, all patients must have at least one of the qualifying conditions to apply for a medical marijuana card.
