How Does It Work?

To obtain a medical marijuana registration card, you need to meet with a qualified physician to discuss your condition. The doctor will help to determine if medical cannabis is a good option for you.

The process includes:

Step 1. Call to make an appointment

  • Meet with our physician to discuss the use of medical cannabis and alternative care for the treatment of your condition. This is an important first step in the application process.
  • Establish a bona-fide physician-patient relationship, and receive a complete physical examination. Patients should bring any previous treatment medical records for review.
  • Continue to manage treatment plan, and assess eligibility for medical cannabis certification.
  • Complete and sign application.
  • Submit application and pay any state-mandated fees. We charge a non-refundable fee of $350, with a reduced fee of $260 for veterans or persons enrolled in federal Social Security Disaibility Income (SSDI) or Supplemental Security Income (SSI) disability program. Veterans, please include a copy of your DD214. SSDI/SSI recipients, please include a copy of your benefit verification letter, dated within the last year.
  • Patient must provide proof of residency. You will need two items that prove you live in Illinois. The addresses on each of the documents must match the address on your application. Bank statements, utility bills, state ID, driver’s license, and voter ID cards are all acceptable. Check the application for a full list of accepted documents.
  • Patients must provide proof of age and identity. Patients must have a copy of a valid, unexpired government-issued photo ID.
  • Veterans – Send in a copy of your DD214 along with the $50 application fee. If you are receiving care at a Veterans Affairs (VA) facility, you may submit medical records from the VA pertaining to the treatment for your qualifying debilitating medical condition from the past year, instead of a Physician Written Certification Form.
  • Wait for the result of your medical cannabis evaluation.
  • Pick up your medical cannabis card.

Step 2. Follow up with your primary care physician, and the appropriate referring specialist, in addition to PT, rehab, etc.

  • Provider will recommend diet and activity advice based on the patient’s medical condition.
